The Fremantle Mental Health Services General Practice Liaison
Service is lead by a GP Liaison Nurse. The prime focus is improvement in patient
outcomes through promotion of communication channels between GPs and Fremantle
Mental Health Service and the collaborative development of protocols for
the referral, consultation, admission and discharge of mental
health patients.
