Enhancing communication with GPs is a priority at Fremantle Hospital and Health Service.  GP Liaison Consultants are attached to clinical directorates at Fremantle Hospital, working one session per week.  The over riding goal is to:

• enhance continuity of care for patients across the primary and tertiary sector;
• facilitate and improve communication, access and utilisation of hospital services;
• GP advocacy; and
• promote collaboration

Feedback from GPs to our GP Liaison team is both welcomed and encouraged.
