GP Liaison

GP Liaison Used with permission St Vincents Health Enhancing communication with GPs is a priority at Fremantle Hospital and Health Service. GP Liaison Consultants are attached to clinical directorates at Fremantle Hospital, working one session per week. The over riding goal is to: enhance continuity of care for patients across the primary and tertiary sector; facilitate and improve communication, access and utilisation of hospital services; GP advocacy; and promote collaboration Feedback from GPs to our GP Liaison team is both welcomed and encouraged.
