The Moss Street Centre is located at 33 Moss Street, East Fremantle 6168. The Moss Street Centre runs the following clinics:
- General Geriatric
- Memory Evaluation- this clinic principally assesses individuals with mild cognitive problems or complex problems that cannot be dealt with in the general geriatric clinic
- Balance & Mobility
- Parkinson’s
- Continence
- Allied Health :
- Clinical Psychologist
- Nurse
- Speech Therapy
- Physiotherapy
- Dietician
- Occupational Therapy
Eligibility for an ACAT Assessment
Patients who are eligible for an ACAT assessment need to:-
- Be Aged! 70 years or more or 50 years or older ATSI (there are occasional exceptions)
- Be Medically Stable!!! Those patients who are receiving acute care or engaged in a rehabilitation program are not eligible ACAT clients.
- Have significant physical, medical, social or psychological care needs that cannot be met more appropriately through other services
- Be a voluntary patient. This standard is not negotiable, involuntary patients under psychiatric care cannot be seen for assessment for residential care.
- Not be eligible for assistance through Disability Services Commission. Written evidence of this from Disability Services is needed for younger disabled clients prior to DCGM considering an ACAT assessment
- Give consent for an ACAT assessment.
