Compliments and complaints about health services

If you wish to compliment, comment on, or complain about, any service received in a public hospital, you should first raise your concerns with attending hospital staff either verbally or in writing. This should be done as soon as you are able to do so.

Many public hospitals in Western Australia have a complaints coordinator or a patient liaison officer (external site) with whom you can raise your concerns. You can also contact hospital management or the Chief Executive/ Executive Director of the health service about your complaint.

If your complaint can’t be resolved by the hospital, you can contact the Health and Disability Services Complaints Office.

Health and Disability Services Complaints Office (HaDSCO)

The Health and Disability Services Complaints Office (HaDSCO) is an independent statutory authority offering a free and impartial resolution service for complaints relating to health, disability and mental health services in Western Australia and the Indian Ocean Territories.

WA Health medical records

Under the Freedom of Information Act 1992 (external site) you can access your WA Health medical records.

Contact the hospital or health service directly. Applications must be made in writing and there is no fee for requesting your records.

Clinicians have an ethical, professional and legal duty to keep patient information confidential.

Medical records from private practices

These are owned by the medical practitioner (if it is a solo practice) or the practice (if the doctor is part of a corporate medical practice). WA Health cannot help with these. You may wish to contact the Australian Health Practitioner Regulation Agency (external site) which supports the Medical Board of Australia (external site).
