The GP Liaison team seeks to improve the patient journey and outcomes through enhanced integration of primary and tertiary health services.

GP Liaison assists health care providers by:

  • Acting as a conduit of information between primary and tertiary health care providers
  • Strengthening links between primary and tertiary care through collaboration and communication
  • Advocating for primary care providers within the tertiary sector

Our team is available to both community GPs and FHHS staff to resolve issues surrounding the transition of patients between primary and tertiary health care services. We strive to improve the systems and processes impacting transition.

GP Consultants are attached to clinical directorates at Fremantle Hospital, Fremantle Mental Health Service and Kaleeya Hospital, and are supported by Project Officers.

Please note this is not a clinical service. We cannot provide clinical advice or assist with employment queries.

Complaints or concerns regarding patient care should be directed to the treating medical team. Should the need arise a formal complaint may lodged with Customer Relations.
